Admissions

The application process for St. John Primary (SJP) and St. Theresa Middle (STA) begins December 1st for the following school year. An application packet may be obtained from either school. The following is required for an applicant to be considered for admission to St. John/St. Theresa School:

1. Completion of Application Form
2. Original Baptismal Certificate
3. Church Parish Verification Form (must be completed and given to your pastor who will forward it to the school)
4. Application Fee $25.00 (non-refundable)
5. Report Card and Standardized Test Scores (if applicable)
6. Agreement Questionnaire – Religious Education, Handbook and Fair Work Requirement
7. Readiness form for Pre-Kindergarten and NEW Kindergarten students

General Information

• The pastoral verification is required of all applicants Catholic and Non-Catholic.
• All pre-kindergarten and new kindergarten applicants will be screened for "readiness" in January.
• Applicants for pre-kindergarten and kindergarten are reviewed and notified of admission results in February.
• All other grade level applications (1st-8th) are reviewed and held until in-school registration and retentions are final.
• Tuition/Fees
• Please note: NO APPLICATION is considered until it is received and ALL of the above is in the applicant's file in the office.
